As a Physical Therapist, you will play a crucial role in providing exceptional care and rehabilitation services to patients, helping them regain mobility, manage pain, and improve their quality of life. Your expertise will contribute to our commitment to delivering personalized and effective therapy programs.
Responsibilities:
– Assessment and Evaluation: Conduct comprehensive assessments to determine patients’ functional abilities, limitations, and goals.
– Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities.
– Implement and administer therapeutic exercises, manual therapy, and other interventions to improve patients’ mobility, strength, flexibility, and function based on each individual patients need.
– Monitor and document patients’ progress, adjusting treatment plans as necessary to achieve optimal outcomes.
– Educate patients and their families about therapy procedures, home exercises, and injury prevention techniques.
– Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.
– Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries.
– Stay updated on advancements in physical therapy techniques, technologies, and best practices, integrating them into clinical practice as appropriate.
– Expected to meet the needs of having patient execute Plan of Care including but not limited to following up with patients if non compliance occurs.
– Be a team player to utilize non clinical staff to support needs of the caseload
– Proper delegation and use of PTA when applicable.
– Be an active participant in helping to drive a collaborative culture based on our service vision
– Ability to adapt to change in patient schedules and daily patterns.
– Adhere to ethical and professional standards, ensuring compliance with regulatory requirements and company policies.
Essential Job Functions:
– Perform comprehensive patient assessments to determine therapy needs and establish treatment goals.
– Develop and implement individualized treatment plans to address patients’ specific conditions and functional limitations.
– Administer therapeutic exercises, manual therapy, and modalities effectively to promote recovery and improve patients’ physical well-being.
– Communicate effectively with patients, families, and healthcare team members, providing education and support throughout the rehabilitation process.
– Maintain accurate and organized documentation of patient progress and treatment outcomes.
QUALIFICATIONS:
- Valid state licensure as a Physical Therapist
- Doctorate or Master’s degree in Physical Therapy from an accredited program
- CPR certification and any other relevant certifications as required by state regulations
- Strong clinical skills with experience in orthopedic, neurological, or geriatric physical therapy (preferred)
- Excellent communication and interpersonal skills
- Ability to work effectively both independently and collaboratively within a team environment
- Proficient computer skills for documentation and record-keeping purposes
Physical Requirements:
– Mobility: Ability to move around freely to assist patients and therapists.
– Lifting: Capable of lifting and carrying equipment or assisting patients as needed (up to 50 pounds).
– Standing: Extended periods of standing and walking during therapy sessions.
– Manual Dexterity: Proficiency in handling therapy tools and equipment.
– Visual and Auditory Skills: Ability to observe patients and communicate effectively.
– Flexibility: Adaptability to changing tasks and schedules in a dynamic healthcare environment.
